Some coach wants to affiliate my son to his team. What does this mean and what should I expect?

The coach that wishes to affiliate your child must fill out the ODMHA player affiliation form.

When do I need to complete an Injury Report?

An Injury Report must be completed when:

  • A player required the assistance of a trainer due to an injury which results in the course of a hockey event
  • A player is taken by ambulance to a hospital
  • If the injury will not allow the player to return to active duty based on a physician’s diagnosis
  • If the possibility exists that there could be any expenses related directly to the injury or if a possible claim/suit could result from the injury against the HC. insurance program

It is the Team Management’s Responsibility to file claims and reports.


I am selling my existing home but do not have a new home confirmed at time of Registration.Can I still register in the new assoc

No, You must have your new residency confirmed prior to registration. You need to speak to the Association President or Association Registrar where you are moving for the exact rules & procedures/documentation requirements of that Association.

Please verify the exact documentation required with the new association.


What do I do if my transfer request is refused by the home Association/District?

You can appeal to the ODMHA by delivering a cheque ($250.00) and the completed transfer form to our office.

My son wants to play in a team out of our immediate region, how do I go about it?

Between Two Different Districts - Competitive and House league

You must be registered in your home association to obtain a Transfer

1. Transfers must be signed by accepting association President, and Chairperson first.

2. Transfers must be signed by home association President, and District Chairperson.

NOTE All Competitive transfers BETWEEN DISTRICTS must be appealed to ODMHA office with a cheque for 250.00 dollars to arrange for a transfer hearing.

Within the same District

Transfers must be signed by both Association Presidents and District Chairperson with every one in agreement


Where does my son/daughter register for competitive tryouts?

Most associations send their members information in the pre-season concerning tryout and conditioning.

Non-members should contact their home association or District Representative for a list of upcoming sessions.

All ODMHA District and Association Representatives are available on this Website.


How do I register my son or daughter to play minor hockey in the Ottawa Area and what documentation do I require?

It is best to contact your local District Chairman. He or she will direct you to the current registrar for your area. To determine the area in which you live and the name and number of the person you should contact click here. You will be required to complete (at registration) a declaration of Residency form** indicating your new address and date of the move to the new address. The new association will also ask you for proof of address, i.e. copy of deed, copy of rental agreement, etc. If you are moving into the ODMHA from another Branch, i.e. moving to ODMHA from Toronto, Quebec, Winnipeg, etc. you must have a letter from your previous association stating that your child has played in their association the past season and is moving to the ODMHA without any prejudice, i.e. no outstanding fees or suspensions.
